| Attachment Upload |
The attachment upload panel allows public, providers and billing agents to browse, select and upload attachments against submitted enrollment applications, claims attachments or member transaction updates. Selecting an attachment description that best fits the file is required before the user is able to browse, select and upload an attachment.
Attachment types permitted are: TIF, TIFF, JPG, JPEG and PDF, which must be less than 20 mb in size.

| Browse | This button allows the user to navigate and select a local attachment to upload. |
| upload attachment | This button initiates the upload process. |
| Upload | Displays the file the user wishes to be uploaded. |
| Attachment Description | Displays the description of the attachment to be uploaded against an application. (Read-Only) |
| Member First Name | Displays the member's first name where documentation is being uploaded against. (Read-Only) |
| Member ID | Displays the member's Georgia Medicaid ID where documentation is being uploaded against. (Read-Only) |
| Member Last Name | Displays the member's last name where documentation is being uploaded against. (Read-Only) |
| Status | Displays the status of the attachment to be uploaded against an application. (Read-Only) |
| Upload Document | Displays the description of the document to be uploaded on behalf of the member. (Read-Only) |
