The form submitted is pending for the following reason (s):

-	Need proof of income between and of nurse aide position (copy of one check stub or W-2 form) If no proof is provided, you will need to retest and/or retrain and retest to update your certification. Must have at least 8 hours of work history within each two-year renewal period.

If there is a change of address, it is the sole responsibility of the CNA to report this change. The CNA has 10 working days to report the change so that the registry will be updated appropriately.
