The form submitted is pending for the following reason (s):

-	Need proof of income between 08/09/2021 and 08/09/2023 of nurse aide position (copy of one check stub or W-2 form) If no proof is provided, you will need to retest and/or retrain and retest to update your certification. Must have at least 8 hours of work history within each two-year renewal period.
-	The transcripts from IRS does not give complete name of employer. Please provide this information in order for the registry to verify the employment was for CNA work.

If there is a change of address, it is the sole responsibility of the CNA to report this change. The CNA has 10 working days to report the change so that the registry will be updated appropriately.
