Healthcare Facility Regulation Division (HFRD) Application Migration

The Georgia Department of Community Health (DCH)'s (HFR) division will be migrating all licensing activities for Private Home Care Providers from the current Georgia MMIS (GAMMIS) system to the new Georgia Healthcare Accreditation and Licensing Electronic System (HFRDGHALES). Please complete all applications before migration.
